Insurance Waiver submission form trouble

NOTE:

As stated by the Dean of Students office, the Medical Insurance (SHIP) waiver is not available till May 1st of every year.
Students on leave will not be able to use the SHIP Waiver form as insurance from the college isn't available to them.
International Students are not allowed to opt out and should contact the Dean of Students if additional support is needed.

What may cause trouble entering information into the SHIP Waiver at https://www.gallagherstudent.com/cuc.Pomona ?

Enter your email address in your username format (which has the format of 'abcd1234@mymail.pomona.edu').

The "Last Name:" information in the Ascension database might be much different from what is seen in other Pomona College information systems. If you encounter someone who has viewed the information in this article and still encounters trouble, please forward the call/report to the Dean of Students office at 909-621-8017.
